Application Process

Institutions should indicate their course of choice and how they think the course will be of benefit to them when submitting their applications. Applications should be submitted at least one month prior to the date when the applying institution would like to have the course conducted.

The SPH-CDC HIV/AIDS Fellowship Program reserves the right to offer the course applied for at another date other than the one specified in the application or to reject the application altogether. Either way, institutions will be informed of the decision taken within a reasonable period of time.

 

Institutions applying for one of the courses identified above should complete an application form Please click to download the form and submit by email to fp@musphcdc.ac.ug, or by post (please indicate: "Application for SPH-CDC Short Course" on the envelope) to the: The Dean, Makerere University School of Public Health, P.O. Box 7072, Kampala. Applications can also be hand-delivered at SPH-CDC offices Ground Floor, Makerere University School of Public Health building, New Mulago Hospital Complex, Kampala.

Institutions should submit the following information along with the filled in application form:

  1. Short profile of the organization applying for the course
  2. Rationale for the course applied for
  3. List of participants (names, qualifications, and positions in the institution)
  4. Budgetary estimates for the training
  5. Contribution from the institution applying for the course
